Instead of escaping processing instructions in the form <code>&lt;? ...
?&gt;</code>, remove it out-right.  This may be useful if the HTML
you are validating contains XML processing instruction gunk, however,
it can also be user-unfriendly for people attempting to post PHP
snippets.
